Polycystic ovary syndrome (PCOS) is the most common hormonal disorder in women of reproductive age, affecting 1 in 10. If you have PCOS and are considering egg freezing, you probably have two questions: will it work? And is it safe?
The short answers: yes, it can work very well — and with the right protocol, it's safe.
Women with PCOS typically have a higher ovarian reserve than average — more antral follicles, often a higher AMH. This means more eggs available for stimulation. In an egg freezing cycle, more eggs means more embryos down the line, and higher cumulative chances of success.
The complication: that same high follicle count puts women with PCOS at higher risk of ovarian hyperstimulation syndrome (OHSS) — a potentially serious side effect where the ovaries overrespond to stimulation hormones. OHSS ranges from mild (bloating, discomfort) to severe (hospitalisation in rare cases).
Experienced clinics adapt the stimulation protocol significantly for women with PCOS. Lower starting doses of FSH, closer monitoring with more frequent ultrasounds, and the use of a GnRH agonist trigger instead of the standard HCG trigger all substantially reduce OHSS risk. The "freeze-all" strategy — freezing all eggs or embryos from the cycle and doing a frozen transfer later — is now standard practice for PCOS patients, as a fresh transfer can worsen OHSS symptoms.
